FEES   
Please note that we are a Private Billing practice.
 
Consultation Fee:
  • Please Check with Reception for the current Consultation Fee. There is a Medicare rebate for eligible patients. 
  • Payment for your consultation is required on the day of service.
  • DVA Card Holders and National Immunisation Programme (NIP) Vaccinations are Bulk Billed.
Procedures:  Additional fees may apply for Procedures such as removal of lesions, biopsies and Implanon.

Please speak to our reception staff if you have any queries about fees.
 
If your consultation is likely to be complex or you have several issues to discuss, please advise reception so we can make arrangements.
                                                                                                                                                                      
  
Booking Appointments
 
Booking appointments can be done in person, over the phone, via our online booking service HEALTH ENGINE, or on our website www.parkviewfamilypractice.com.au
 
If you are a new patient you will be requested to fill out a new patient information form which contains a series of personal identifying questions: DOB, Gender, Ethnicity, Phone, Address, Medicare Number & Concession Card details. The form also collects information such as who we should contact in an emergency, your consent for SMS reminders, your consent to staff phoning or writing to you regarding results, follow-up, recalls and reminders, personal history including allergies.  From time to time we will check these details with you to ensure our information is current and correct.   Please notify reception if your details have changed.
We also request photo-ID for registration of new patients at the initial consultation (for children, parental ID is acceptable).

 

Follow-up of Results
If your doctor requests tests or refers you to a specialist, you will be requested to make a follow-up appointment to discuss your results, and further treatment and options if necessary.  If unsure, please ask your doctor at the time of the visit when to re-book.
Please note: No results will be given out over the phone by reception staff.  If your doctor has asked for you to phone for your results, these must be given to you by your doctor.
 
Communication Policy
All requests and test results need to be discussed at a phone appointment with the Doctor. A doctor is available during regular surgery hours for emergency advice.
If the Doctor has requested that you phone him/her, a message will be taken for the Doctor to return your call.
 
Please note:  It is the practice policy that we do not accept electronic communications from patients.
 
Telephone Requests for Prescriptions
Script requests for a regular medication may be taken over the phone or via our website www.parkviewfamilypractice.com.au for a fee of $25. 
Please note that in some circumstances the Doctor may need to see you before prescribing a medication.
 
Privacy
Our practice collects and securely stores personal health information, and safeguards its confidentiality and privacy in accordance with Australian Privacy Laws.
 
Feedback
You are welcome to provide Feedback on any aspect of our Practice. Please leave your comments in a sealed envelope for Melissa, our Practice Manager.
 
Complaints
If you have a complaint to make, please address it to Melissa, the Practice Manager in a letter. 
If you would like to make an external complaint, please contact the Health and Disability Services Complaints Office
on 6551 7600, or you can request a complaint form from us.  We request that you attempt to resolve your complaint with us first.
